Program Rules
Team Surgical (“Sponsor”) Refer A Friend Program (Program) allows you (“Referrer”) to encourage friends (“Friends”) to purchase a defibrillator(s) from Team Surgical.
By participating in the Refer a Friend program, you acknowledge that you have read, understood, and agree to be bound by these Program Rules.
1.Eligibility: As a defibrillator customer or associate of Team Surgical, you are eligible to receive a referral fee for each new defibrillator customer you send to Team Surgical. Team Surgical reserves the right to disapprove of a referral customer if it determines that any of the following conditions apply; i) the referred customer is a pre existing customer ii) the customer is managed/owned by a third party already doing business with Team Surgical iii) the referred customer is ineligible due to other circumstances such as governmental employees prohibited from accepting referral fees, physician’s accepting referral fees prohibited under the federal Anti-Kickback Statute or any other legal hindrances iv) the referred customer has a contract or is part of a buying group that enables them to obtain pricing which would make this program not feasible.

Any applicable taxes owed by the “Referrer” are the responsibility of the “Referrer”. Federal Income Tax must be claimed and may be due on amounts exceeding a certain dollar amount. Please consult a CPA for guidance.

The “Referrer” will be paid a referral fee for each defibrillator referred customer purchases from Team Surgical for a period of one year from original purchase. There is no time limit from original contact to the original purchase.
